GENERAL PURPOSE RELOADABLE PREPAID VISA CARD AGREEMENT

This reloadable card Agreement (this “Agreement’) governs your and our rights and responsibilities concerning this reloadable card. Please read the Agreement carefully and keep a copy for your records. By accepting, activating or using your Card, you are agreeing to this Agreement.
Definitions: In this Agreement, the plural form of a word also means the singular, and vice versa. “You,” “your,” “yours” or “Cardholder” means the person who has received this Card. “Tulsa FCU,” “Credit Union”, “we,” “us,” “our,” or “ourselves” means Tulsa Federal Credit Union, the Credit Union providing this Agreement to you. “Account” or “Card” means the reloadable prepaid Card issued by us.
Card Qualifications: You must be a Credit Union member to purchase VISA Prepaid Cards from us. Cards may only be shipped within the United States. You must be a resident of the United States and at least 18 years or older to purchase the Card.
Before Using Your Card: Activate and register your Card by visiting www.tulsafederalcu.org. From our homepage, choose Prepaid Visa Debit Cards under Products, and then click on Register Card next to the image of your Card type. Read and accept the Disclosure and Terms and Conditions to be directed to our Prepaid Card site. Under Cardholder Login, select “Need a Login.” Fill in the information and select Continue. After the Card is registered, select the Activate Card option in the Account Activities dropdown menu. A Personal Identification Number (PIN) will be assigned once activation is confirmed. You may also call 866.760.3156 to activate your Card and obtain your PIN. You will have the option to select your own PIN, via phone only, if you choose to do so.
Using Your Card: The Card will be personalized with the cardholder’s name when purchased through our website at www.tulsafederalcu.org or at any of our branches. The Personalized Card should be received in the mail within 7-10 business days from the date of completed purchase. The address listed for the primary cardholder must match the billing address for the debit/credit card used to purchase the Prepaid Card on the website. You may use any of our debit or credit cards to purchase or fund a Prepaid Card through the website.
Please sign the back before you use the Card. You are responsible for keeping the Card information, including the Card number, PIN, expiration date, and Card Verification Value code confidentially contained within your records. Do not write your PIN on or near the Card. Upon activation of the Card, for your protection and for regulatory purposes, we may request, and you agree to provide, some additional identification verification information such as your home phone number, date of birth, social security number, and zip code. We may use this data for a range of purposes, including but not limited to facilitating refunds if your Card is lost or stolen, enhancing usage at merchants that may require zip code authorization, and aiding in collection efforts in the event of a negative balance. If you do not wish to provide the data, or if the data you provide us cannot be appropriately verified, we reserve the right to refuse or cancel your Card, and reimburse you for the initial load amount. If the data you provide to us changes, such as an address change, we reserve the right to block access to your funds until your information has been updated. If you fail to update your information, we reserve the right to cancel your card. We may suspend your Card if we notice excessive use of your Card or other suspicious or potentially illegal activities. Access will be reinstated once we have notified you, clarified the situation, and rectified any problems.
Personalized Cards have an Allowance Feature that permits automatic funding to the Card from pre-existing stored payment methods on a weekly, bi-monthly, or monthly basis. To activate the Allowance Feature, you must register your Card at www.tulsafederalcu.org, and follow the instructions for activating the feature.
The Card is non-transferable. Please do not give your Card to anyone else to use. The primary cardholder, the Credit Union member, can order online up to 2 secondary Cards for family members or companions at the time of purchase. Secondary Cards are personalized with the cardholder’s name when ordered from our website or when purchased at a Credit Union branch. When purchasing a secondary Card for a young person with no card experience, be sure to go over all of the materials that accompany the Card. Secondary Cards will utilize the same prepaid balance as the primary Card for all transactions and fees. These Cards will have to be separately activated, and a separate PIN will need to be requested for each Card. Secondary Cards will function under the same terms and conditions as the primary Card. You are responsible for all authorized transactions made with the secondary Card. Only the primary cardholder, Credit Union member, can load funds to the account or access the Card information on our secured website. Secondary cardholders may request information about the balance and transaction history on their Card by calling Cardholder Services at 866.760.3156.
Your Card is issued by us, pursuant to a license from VISA U.S.A., Inc., and may be used for purchases at most merchants where VISA Debit is accepted. Your Card allows you to access your prepaid funds at ATMs that display the VISA brand, and merchants who accept VISA cards worldwide. The Card can be processed as a Credit or Debit transaction. When paying as a Credit transaction, you may sign a sales receipt. When paying as a Debit transaction, you will need to enter your PIN. Some merchants may allow you to use your Card for cash-back on purchases, subject to the amount available on the Card, and cash may be advanced at a VISA affiliated financial institution (both subject to limits).Your Card is neither a credit card, nor a deposit account insured by the NCUA. The Card may not be used for recurring payments, non-financial money exchange establishments, illegal transactions, and gambling establishments.
You do not have the right to stop payment on any purchase transaction originated by the use of the Card. We are not responsible for any injury to you or to anyone else caused by any goods or services purchased or leased with your Card. If you are entitled to a refund for any reason for goods or services obtained with a Card, you agree to accept credits to your Card in place of cash.
When you use your Card at an automated gas station pump you will require a minimum of $99 available credit on the Card to enable gas to be dispensed (It may take up to 24 hours before any unused amount is available to spend due to the way in which the transactions are processed.) If your available balance is less than this, please pay the attendant in the gas station. If you use your card to make a telephone call this can require a minimum credit of $15. If you use your Card to purchase meals or drinks many restaurants and bars factor in a tip into the price of up to 25% when pre-authorizing Card purchases, but you will only be debited with the actual amount you spend. Should you use your Card to pre-authorize a hotel reservation, automobile rental, or cruise ship business, add 25% to the reservation. Any amount requested in a pre-authorization will not be available to you to spend using the Card, although you will only be finally debited with the actual amount you spend. If 30 days elapse without you authorizing an amount, any pre-authorization will be cancelled and this will be available to you. Mail order companies often add a similar amount for potential shipping costs, although the final bill will be the amount debited.
The minimum load value is $20.00 and the maximum load value is $3000.00. The maximum value on the Card may not exceed $3,000.00 at any time. You may use it as often as you like while there is a balance remaining on the Card, and only up to the amount left on the Card. There is no minimum purchase amount. Purchases and transactions (including any transaction fees) will be deducted from your Card balance until the value reaches zero. If a transaction initiated by you exceeds the remaining balance on the Card, you must inform the merchant before completing the transaction. Any transaction attempted for more than the amount available on the Card may be declined. Therefore, you must know the amount available on your Card, and inform the merchant to process the transaction in that amount. You must pay the difference with another form of payment. If you are given value through the use of the Card greater than the balance remaining, you will pay us on demand the amount by which your transactions exceeded the balance remaining on the Card. You can check the balance or remit a payment by calling 866.760.3156 for balances and transaction history, by writing to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101-9939 and include your account or card number and payment amount, by visiting one of our branches, or by visiting www.tulsafederalcu.org.
In the event that you need to return an item you purchased with your Card, the merchant will handle the item in accordance with VISA guidelines. The merchant may credit your Card (put the funds back on the Card) thereby increasing your available funds. The funds may take up to 7 business days to be credited to your Card.
Fees and Charges: The following account fees will apply as defined below:
• Purchase/Enrollment Fee – There will be a fee of $4.95 added to the load value on the Card so that the balance upon receipt of the card will not include that fee (for example, if a $100.00 Card is purchased, the amount charged to the purchaser’s account is $104.95 upon purchase and
the Card balance is $100.00).
• Additional Card Fee – You may add up to 2 additional Cards at the time of enrollment to draw from the same funds for $4.95 each.
• Reload Fee – There will be a fee of $2.00 each time the card is reloaded. The Card may be reloaded at one of our branches or on the website at www.tulsafederalcu.org, or by calling our Member CARE Center at 800.256.5626. The fee is not affected by the number of Cards associated with the prepaid funds.
• ATM Transaction Fee (Domestic) – There will be a $1.50 transaction fee for each ATM withdrawal. The first 2 withdrawals each month are free. ATMs not owned by us may be subject to an additional surcharge, which is controlled by the owner of the machine and cannot be waived by us. If there is a surcharge, it will be deducted from your prepaid Card balance.
• ATM Transaction Fee (International) – There will be a $2.50 transaction fee for each ATM withdrawal. The first 2 withdrawals each month are free. ATMs not owned by us may be subject to an additional surcharge, which is controlled by the owner of the machine and cannot be waived by us. If there is a surcharge, it will be deducted from your prepaid Card balance.
• Foreign Transaction Fee – For transactions not originating in the United States or in foreign currencies, a fee of 1% of the converted transaction amount will be applied to your purchase.
• Lost/Stolen/Replacement Fee – There will be a $10.00 fee to replace a lost/stolen/damaged card. Your Card must be registered through the website to qualify for a reissue.
• IVR (Interactive Voice Response) Call Fee – a fee of $1.00 will be charged to the Card balance for each time a call to the IVR system is made. The first two calls per month are free.
• Customer Service (Live Agent) Fee – a fee of $3.00 will be charged to the Card balance for each call to a live customer service agent. The first call each month is free of charge.
• Inactivity Fee – An Inactivity Fee of $3.50 will be charged per month after 365 days of inactivity until the card balance reached zero.
• Express Delivery Fee – An Express Delivery Fee of $25.00 will apply to any Card that must have a rushed delivery when through the website or at a branch.
Card Limits. The daily spending limit for point-of-sale transactions at merchants is $3,000.00. You may not exceed more than 15 purchase transactions per day, up to the balance remaining on your Card.
• You are allowed up to 3 ATM withdraws per day, the total amount is not to exceed $1,530.00 at domestic ATMs or $1830.00 at international ATMs. The minimum ATM withdraw amount is $10.00.
• You are allowed up to 3 over the counter cash advances per day with the total amount not to exceed the balance on your card. The minimum over the counter cash advance is $25.00.
• Your Card is restricted to 4 value loads in a 7 day period. The total sum of value loads within that 7 day period is not to exceed $12,000.00.
• You may add up to 2 additional Cards for access to the same prepaid funds at the time of initial load.
• Personalized cards have an unlimited number of reloads with the exception of the 7-day restriction mentioned above.

Notice of Fees Charged by Others: If you use an ATM that we do not own, the owner of that ATM, or the network that processes your transaction, or both, may charge you a fee. Any such fee, which is generally called a “surcharge,” is not a fee that we charge you; however, we are authorized to pay any such surcharge from your account. Any such surcharge is in addition to any fees that we may charge you.
Non U.S. Transactions. If any Card transaction is made in a currency other than that loaded on the Card, the amount will be converted into the appropriate currency at an exchange rate on the day the transaction is processed. The exchange rate used to determine the transaction amount in US dollars is either a rate selected by VISA from the range of rates available in wholesale currency markets for the applicable central processing date, which may vary from the rate VISA itself receives, or the government mandated rate in effect for the applicable central processing date.
Liability for Unauthorized Use – Lost or Stolen Cards. If you believe the Card has been lost or stolen, or that someone has accessed or may access money from the Card without your permission, contact Cardholder Services immediately at 866.760.3156 or our Care Center at 800.256.5626 or 918.610.0200, or write to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK 74101, or visit one of our branches. You will not be liable for lost value on the Card if you satisfy the following conditions:
• You have immediately called us and reported the Card lost or stolen, which allows us to the status the Card.
• You have signed the Card on its reverse side in permanent ink.
• You inform us of the Card number and the approximate date of your last authorized use.
• You have not reported two or more incidents of unauthorized use to us in the preceding 12-month period.
• You report all facts of the loss or theft to us and you cooperate in our investigation.

If you permit someone else to use your Card, we will treat this as if you have authorized this person to use your Card and you will be responsible for any transactions initiated by said person.
Our Liability for Failure to Make Transactions. We may restrict access to the Card, temporarily or permanently, if we notice suspicious activity in connection with the Card, and we will notify you if we do so. We have no liability for restricting access to the Card because of suspected suspicious activity. Access will be reinstated if we determine that there has been no unauthorized use of the Card.
If we do not complete a transfer from the Card on time or in the correct amount according to our Agreement with you (other than for restrictions because of suspicious activity), we will be liable for the correct amount of the transfer. However, there are some exceptions. We will not be liable, for instance:
• If through no fault of ours, a merchant refuses to honor the Card;
• If through no fault of ours, you do not have enough money available on the Card to make a purchase;
• If the terminal or system was not working properly;
• If the Card is expired, damaged, or revoked;
• If the transaction information supplied to us by you or a third party is incorrect or untimely;
• If the Card has been reported lost or stolen;
• If we have reason to believe there is something wrong, for example, that the Card has been stolen;
• If circumstances beyond our control (such as flood or fire) prevent a transaction, despite reasonable precautions that we have taken;
• The merchant authorizes an amount greater than the purchase amount.

There may be other applicable exceptions as otherwise provided by state or federal laws.
PIN Protection. You agree to take all necessary steps to protect your PIN and never disclose your PIN to anyone. For security purposes, never write your PIN on the Card and never carry a record of your PIN on your person. If you permit someone else to use your Card and you give that person your PIN, we will treat this as if you have authorized this person to use your Card at their discretion for that transaction and any future transactions. You will be responsible for any transactions initiated by such person.
Your VISA Reloadable Prepaid Card will only allow 3 incorrect PIN tries in one day before the security system will lock you out, preventing access to any funds. The Card will remain locked for a 24 hour period. You may change your PIN after activation by calling Cardholder Services at 866.760.3156, or by visiting our website at www.tulsafederalcu.org.
Transaction Activity. A transaction activity report, which includes your balance and details the use of your Card, is available by calling Cardholder Services at 866.760.3156, writing to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101, visiting our website at www.tulsafederalcu.org, or visiting one of our branches. The transaction activity report must be requested by the Cardholder. The information is available to you 24 hours a day, 7 days a week. Your Card account number and original load value is required for authentication.
Error Resolution Procedures. In case of errors or questions about transactions arising from the use of your Card, or if any records you review show transactions that you did not make, call Cardholder Services at 866.760.3156, write to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101, visit our website at www.tulsafederalcu.org, or visit one of our branches immediately. We must hear from you no later than 60 days after the transaction appeared on the transaction activity report. We may, at our discretion, extend the period for a reasonable time. In order to help you with your questions, we will need the following information:
• Your name, address, phone number, and Card number.
• A description of the error or transaction you are unsure about and why you believe there is an error or why you need more information.
• The dollar amount and date of the suspected error.

Once the type of error is identified, we will mail you the appropriate paperwork to complete. No other action will be taken until the completed paperwork is received at the address specified in the documentation. Upon receipt of the completed paperwork, we will send you a confirmation letter and begin the investigation. We will determine whether an error occurred within 60 days. Our business days are Monday through Friday, and exclude holidays. If we need more time, however, we may take up to 45 additional days to investigate your complaint or question. If we decide to do this, we will send you a letter. Credit will be given only after it has been determined that it is warranted.
We will tell you the results after completing our investigation. If we decide that there was no error, we will send you a written explanation. You may ask for copies of the documents we used in our investigation by calling Cardholder Services at 866.760.3156.
Unauthorized Use of Your Card. You will notify us promptly within a reasonable time if the Card or PIN is lost or stolen. A replacement Card will be issued for the balance remaining on the Card at the time it is reported lost or stolen. To report a Card, PIN, or both lost or stolen, call us at 866.760.3156 or 918.610.0200 during business hours, or write to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK, 74101-9939, or visit one of our branches.
A transaction is unauthorized if it is not initiated by you, you did not give permission to make the transaction, and you do not benefit from the transaction in any way. Any unauthorized transactions will be re-credited to your Card in 5 business days. If you do not tell us within 60 days after the transaction was posted to your account, you may not get back any money you lost if we can prove that we could have stopped someone from fraudulently using the Card if you had told us in time.
Special Notice to Visa Cardholders. If there is an unauthorized use of your Visa card that takes place on the Visa network, your liability will be zero ($0.00). This provision limiting your liability does not apply to non-U.S. ATM transactions or transactions using your PIN that are not processed by Visa. Additionally, your liability with respect to unauthorized transactions may be greater than this zero ($0.00) liability limit, to the extent allowed under Visa rules or applicable law. The Visa zero ($0.00) liability limit is subject to certain conditions. If we reasonably determine, based on substantial evidence, that you were grossly negligent or fraudulent in the handling of your card this zero ($0.00) liability limit will not apply. In any case, to minimize your potential liability you should notify us immediately of any unauthorized use but no later than 60 days after the transaction appeared on the transaction activity report. Disclosure of Information to Third Parties. We may disclose information to third parties about your Card or the transactions that you make:
• where it is necessary for completing the transaction;
• to verify the existence and condition of your Card for a third party;
• to comply with government agency or court orders;
• if you give us your consent; and
• as otherwise required by law.

Closure, Expiration, or Revocation of Card. Your Card may be unloaded at an ATM using a PIN, and may be closed by contacting Cardholder Services at 1.866.760.3156, visiting one of our branches, or by mailing a request in writing to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK 74101-9939.
Your Card is valid through the last day of the month of the expiration date displayed on your Card. The funds are available for a minimum of 5 years from the load/purchase date or until the entire prepaid value associated with the Card has been depleted, whichever comes first. If you do not deplete the balance by the expiration date of the Card and it has not been 5 years from the load/purchase date, a replacement card may be issued. (See Liability For Unauthorized Use – Lost or Stolen Cards section for contact information to request a replacement card). A renewal Card will automatically be mailed to the last known address as reflected on the cardholder record in the Card Processing System 30 days prior to the expiration date printed on the front of your Card. You must call Cardholder Services at the number on the back of the Card and activate your renewal Card when it is received. We encourage you to use the entire amount before the funds expire. The funds will expire in accordance with the statutory period under the unclaimed property laws in your state of residence if your address is known to us. When your state of residence is Oklahoma or cannot be identified, funds will expire in accordance with Oklahoma’s Uniform Unclaimed Property Act which has a statutory period of five years. In the event that you terminate your membership with the credit union, your card will be closed and any remaining balance will be refunded to you.
The Card is the property of us and we may revoke the Card at any time without cause or notice. You agree to surrender a revoked Card promptly upon demand. Upon revocation, any stored value remaining on the Card will be refunded to you less any applicable fees.
Amendments: We may change or add new terms to this Agreement at any time, including without limitation, new fees, fee increases or enforcement of rights and obligations under this Agreement. These changes will be posted on our website at www.tulsafederalcu.org, or as required by applicable law.
Assignment: We may at any time and without notice to you assign the Card, this Agreement or any of our rights or obligations under the Card or this Agreement to any person or entity. The person or entity to which we make any such assignment shall be entitled to all our rights and/or obligations under this Agreement, to the extent assigned.
Enforcement: This Agreement or any claim or dispute arising under this Agreement will be construed in accordance with and governed by the laws of the State of Oklahoma, together with specifically applicable federal laws governing this Agreement. If any law or judicial ruling renders any term or condition of this Agreement unenforceable, the remaining terms and conditions will remain in full force and effect.
Governing Law. Tulsa Federal Credit Union is located in Oklahoma and your Card is issued from Oklahoma irrespective of your residency or the jurisdictions in which you use the Card. This Agreement is entered into with you in the State of Oklahoma and shall be governed, construed, and enforced in all respects and all causes of action relating to terms or conditions of Card usage, or terms and conditions of this Agreement according to the laws of Oklahoma, without regard to its internal conflicts of law principles.

Some merchants may allow you to use your Card for cash-back on purchases, subject to the amount available on the Card, and cash may be advanced at a VISA affiliated financial institution (both subject to limits).Your Card is neither a credit card, nor a deposit account insured by the NCUA. The Card may not be used for recurring payments, non-financial money exchange establishments, illegal transactions, and gambling establishments.
You do not have the right to stop payment on any purchase transaction originated by the use of the Card. We are not responsible for any injury to you or to anyone else caused by any goods or services purchased or leased with your Card. If you are entitled to a refund for any reason for goods or services obtained with a Card, you agree to accept credits to your Card in place of cash.
When you use your Card at an automated gas station pump you will require a minimum of $99 available credit on the Card to enable gas to be dispensed (It may take up to 24 hours before any unused amount is available to spend due to the way in which the transactions are processed.) If your available balance is less than this, please pay the attendant in the gas station. If you use your card to make a telephone call this can require a minimum credit of $15. If you use your Card to purchase meals or drinks many restaurants and bars factor in a tip into the price of up to 25% when pre-authorizing Card purchases, but you will only be debited with the actual amount you spend. Should you use your Card to pre-authorize a hotel reservation, automobile rental, or cruise ship business, add 25% to the reservation. Any amount requested in a pre-authorization will not be available to you to spend using the Card, although you will only be finally debited with the actual amount you spend. If 30 days elapse without you authorizing an amount, any pre-authorization will be cancelled and this will be available to you. Mail order companies often add a similar amount for potential shipping costs, although the final bill will be the amount debited.
The minimum load value is $15.00 and the maximum load value is $2500.00. The maximum value on the Card may not exceed $2500.00 at any time. You may use it as often as you like while there is a balance remaining on the Card, and only up to the amount left on the Card. There is no minimum purchase amount. Purchases and transactions (including any transaction fees) will be deducted from your Card balance until the value reaches zero. If a transaction initiated by you exceeds the remaining balance on the Card, you must inform the merchant before completing the transaction. Any transaction attempted for more than the amount available on the Card may be declined. Therefore, you must know the amount available on your Card, and inform the merchant to process the transaction in that amount. You must pay the difference with another form of payment. If you are given value through the use of the Card greater than the balance remaining, you will pay us on demand the amount by which your transactions exceeded the balance remaining on the Card. Box 267, Tulsa, OK 74101-9939 and include your account or card number and payment amount, by visiting one of our branches, or by visiting www.tulsafederalcu.org.
In the event that you need to return an item you purchased with your Card, the merchant will handle the item in accordance with VISA guidelines. The merchant may credit your Card (put the funds back on the Card) thereby increasing your available funds. The funds may take up to 7 business days to be credited to your Card.
Fees and Charges: The following account fees will apply as defined below:
• Purchase/Enrollment Fee – There will be a fee of $3.95 added to the load value on the Card so that the balance upon receipt of the card will not include that fee (for example, if a $100.00 Card is purchased, the amount charged to the purchaser’s account is $103.95 upon purchase and the Card balance is $100.00).
• Additional Card Fee – You may add up to 3 additional Cards at the time of enrollment for $3.95 each. Each card will have its own balance to load onto it.
• ATM Transaction Fee (Domestic) – There will be a $1.50 transaction fee for each ATM withdrawal. The first 2 withdrawals each month are free. ATMs not owned by us may be subject to an additional surcharge, which is controlled by the owner of the machine and cannot be waived by us. If there is a surcharge, it will be deducted from your prepaid Card balance.
• ATM Transaction Fee (International) – There will be a $2.50 transaction fee for each ATM withdrawal. The first 2 withdrawals each month are free. ATMs not owned by us may be subject to an additional surcharge, which is controlled by the owner of the machine and cannot be waived by us. If there is a surcharge, it will be deducted from your prepaid Card balance.
• Foreign Transaction Fee – For transactions not originating in the United States or in foreign currencies, a fee of 1% of the converted transaction amount will be applied to your purchase.
• Lost/Stolen/Replacement Fee – There will be a $10.00 fee to replace a lost/stolen/damaged card. Your Card must be registered through the website to qualify for a reissue.
• IVR (Interactive Voice Response) Call Fee – a fee of $1.00 will be charged to the Card balance for each time a call to the IVR system is made. The first two calls per month are free.
• Customer Service (Live Agent) Fee – a fee of $3.00 will be charged to the Card balance for each call to a live customer service agent. The first call each month is free of charge.
• Inactivity Fee – An Inactivity Fee of $3.50 will be charged per month after 365 days of inactivity until the card balance reached zero.

Express Delivery Fee – An Express Delivery Fee of $25.00 will apply to any Card that must have a rushed delivery when through the website or at a branch.
Card Limits. The daily spending limit for point-of-sale transactions at merchants is $2500.00. You may not exceed more than 15 purchase transactions per day, up to the balance remaining on your Card.
• You are allowed up to 3 ATM withdraws per day, the total amount is not to exceed $1,530.00 at domestic ATMs or $1830.00 at international ATMs. The minimum ATM withdraw amount is $10.00.
• You are allowed up to 3 over the counter cash advances per day with the total amount not to exceed the balance on your card. The minimum over the counter cash advance is $25.00.
• Your Card is restricted to 4 value loads in a 7 day period. The total sum of value loads within that 7 day period is not to exceed $10,000.00.
• You may add up to 2 additional Cards for access to the same prepaid funds at the time of initial load.
• Personalized cards have an unlimited number of reloads with the exception of the 7-day restriction mentioned above.

Notice of Fees Charged by Others: If you use an ATM that we do not own, the owner of that ATM, or the network that processes your transaction, or both, may charge you a fee. Any such fee, which is generally called a “surcharge,” is not a fee that we charge you; however, we are authorized to pay any such surcharge from your account. Any such surcharge is in addition to any fees that we may charge you.
Non U.S. Transactions. If any Card transaction is made in a currency other than that loaded on the Card, the amount will be converted into the appropriate currency at an exchange rate on the day the transaction is processed. The exchange rate used to determine the transaction amount in US dollars is either a rate selected by VISA from the range of rates available in wholesale currency markets for the applicable central processing date, which may vary from the rate VISA itself receives, or the government mandated rate in effect for the applicable central processing date.
Liability for Unauthorized Use – Lost or Stolen Cards. If you believe the Card has been lost or stolen, or that someone has accessed or may access money from the Card without your permission, contact Cardholder Services immediately at 866.901.8090 or our Care Center at 800.256.5626 or 918.610.0200, or write to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK 74101, or visit one of our branches. You will not be liable for lost value on the Card if you satisfy the following conditions:
• You have immediately called us and reported the Card lost or stolen, which allows us to the status the Card.
• You have signed the Card on its reverse side in permanent ink.
• You inform us of the Card number and the approximate date of your last authorized use.
• You have not reported two or more incidents of unauthorized use to us in the preceding 12-month period.
• You report all facts of the loss or theft to us and you cooperate in our investigation.

If you permit someone else to use your Card, we will treat this as if you have authorized this person to use your Card and you will be responsible for any transactions initiated by said person.
Our Liability for Failure to Make Transactions. We may restrict access to the Card, temporarily or permanently, if we notice suspicious activity in connection with the Card, and we will notify you if we do so. We have no liability for restricting access to the Card because of suspected suspicious activity. Access will be reinstated if we determine that there has been no unauthorized use of the Card.
If we do not complete a transfer from the Card on time or in the correct amount according to our Agreement with you (other than for restrictions because of suspicious activity), we will be liable for the correct amount of the transfer. However, there are some exceptions. We will not be liable, for instance:
• If through no fault of ours, a merchant refuses to honor the Card;
• If through no fault of ours, you do not have enough money available on the Card to make a purchase;
• If the terminal or system was not working properly;
• If the Card is expired, damaged, or revoked;
• If the transaction information supplied to us by you or a third party is incorrect or untimely;
• If the Card has been reported lost or stolen;
• If we have reason to believe there is something wrong, for example, that the Card has been stolen;
• If circumstances beyond our control (such as flood or fire) prevent a transaction, despite reasonable precautions that we have taken;
• The merchant authorizes an amount greater than the purchase amount.

There may be other applicable exceptions as otherwise provided by state or federal laws.
PIN Protection. You agree to take all necessary steps to protect your PIN and never disclose your PIN to anyone. For security purposes, never write your PIN on the Card and never carry a record of your PIN on your person. If you permit someone else to use your Card and you give that person your PIN, we will treat this as if you have authorized this person to use your Card at their discretion for that transaction and any future transactions. You will be responsible for any transactions initiated by such person.
Your VISA Reloadable Prepaid Card will only allow 3 incorrect PIN tries in one day before the security system will lock you out, preventing access to any funds. The Card will remain locked for a 24 hour period. You may change your PIN after activation by calling Cardholder Services at 866.901.8090, or by visiting our website at www.tulsafederalcu.org.
Transaction Activity. A transaction activity report, which includes your balance and details the use of your Card, is available by calling Cardholder Services at 866.901.8090, writing to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101, visiting our website at www.tulsafederalcu.org, or visiting one of our branches. The transaction activity report must be requested by the Cardholder. The information is available to you 24 hours a day, 7 days a week. Your Card account number and original load value is required for authentication.
Error Resolution Procedures. In case of errors or questions about transactions arising from the use of your Card, or if any records you review show transactions that you did not make, call Cardholder Services at 866.901.8090, write to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101, visit our website at www.tulsafederalcu.org, or visit one of our branches immediately. We must hear from you no later than 60 days after the transaction appeared on the transaction activity report. We may, at our discretion, extend the period for a reasonable time. In order to help you with your questions, we will need the following information:
• Your name, address, phone number, and Card number.
• A description of the error or transaction you are unsure about and why you believe there is an error or why you need more information.
• The dollar amount and date of the suspected error.

Once the type of error is identified, we will mail you the appropriate paperwork to complete. No other action will be taken until the completed paperwork is received at the address specified in the documentation. Upon receipt of the completed paperwork, we will send you a confirmation letter and begin the investigation. We will determine whether an error occurred within 60 days. Our business days are Monday through Friday, and exclude holidays. If we need more time, however, we may take up to 45 additional days to investigate your complaint or question. If we decide to do this, we will send you a letter. Credit will be given only after it has been determined that it is warranted.
We will tell you the results after completing our investigation. If we decide that there was no error, we will send you a written explanation. You may ask for copies of the documents we used in our investigation by calling Cardholder Services at 866.901.8090.
Unauthorized Use of Your Card. You will notify us promptly within a reasonable time if the Card or PIN is lost or stolen. A replacement Card will be issued for the balance remaining on the Card at the time it is reported lost or stolen. To report a Card, PIN, or both lost or stolen, call us at 866.901.8090 or 918.610.0200 during business hours, or write to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK, 74101-9939, or visit one of our branches.
A transaction is unauthorized if it is not initiated by you, you did not give permission to make the transaction, and you do not benefit from the transaction in any way. Any unauthorized transactions will be re-credited to your Card in 5 business days. If you do not tell us within 60 days after the transaction was posted to your account, you may not get back any money you lost if we can prove that we could have stopped someone from fraudulently using the Card if you had told us in time.
Special Notice to Visa Cardholders. If there is an unauthorized use of your Visa card that takes place on the Visa network, your liability will be zero ($0.00). This provision limiting your liability does not apply to non-U.S. ATM transactions or transactions using your PIN that are not processed by Visa. Additionally, your liability with respect to unauthorized transactions may be greater than this zero ($0.00) liability limit, to the extent allowed under Visa rules or applicable law. The Visa zero ($0.00) liability limit is subject to certain conditions. If we reasonably determine, based on substantial evidence, that you were grossly negligent or fraudulent in the handling of your card this zero ($0.00) liability limit will not apply. In any case, to minimize your potential liability you should notify us immediately of any unauthorized use but no later than 60 days after the transaction appeared on the transaction activity report. Disclosure of Information to Third Parties. We may disclose information to third parties about your Card or the transactions that you make:
• where it is necessary for completing the transaction;
• to verify the existence and condition of your Card for a third party;
• to comply with government agency or court orders;
• if you give us your consent; and
• as otherwise required by law.

Closure, Expiration, or Revocation of Card. Your Card may be unloaded at an ATM using a PIN, and may be closed by contacting Cardholder Services at 866.901.8090, visiting one of our branches, or by mailing a request in writing to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK 74101-9939.
Your Card is valid through the last day of the month of the expiration date displayed on your Card. The funds are available for a minimum of 5 years from the load/purchase date or until the entire prepaid value associated with the Card has been depleted, whichever comes first. If you do not deplete the balance by the expiration date of the Card and it has not been 5 years from the load/purchase date, a replacement card may be issued. (See Liability For Unauthorized Use – Lost or Stolen Cards section for contact information to request a replacement card). A renewal Card will automatically be mailed to the last known address as reflected on the cardholder record in the Card Processing System 30 days prior to the expiration date printed on the front of your Card. You must call Cardholder Services at the number on the back of the Card and activate your renewal Card when it is received. We encourage you to use the entire amount before the funds expire. The funds will expire in accordance with the statutory period under the unclaimed property laws in your state of residence if your address is known to us. When your state of residence is Oklahoma or cannot be identified, funds will expire in accordance with Oklahoma’s Uniform Unclaimed Property Act which has a statutory period of five years. In the event that you terminate your membership with the credit union, your card will be closed and any remaining balance will be refunded to you.
The Card is the property of us and we may revoke the Card at any time without cause or notice. You agree to surrender a revoked Card promptly upon demand. Upon revocation, any stored value remaining on the Card will be refunded to you less any applicable fees.
Amendments: We may change or add new terms to this Agreement at any time, including without limitation, new fees, fee increases or enforcement of rights and obligations under this Agreement. These changes will be posted on our website at www.tulsafederalcu.org, or as required by applicable law.
Assignment: We may at any time and without notice to you assign the Card, this Agreement or any of our rights or obligations under the Card or this Agreement to any person or entity. The person or entity to which we make any such assignment shall be entitled to all our rights and/or obligations under this Agreement, to the extent assigned.
Enforcement: This Agreement or any claim or dispute arising under this Agreement will be construed in accordance with and governed by the laws of the State of Oklahoma, together with specifically applicable federal laws governing this Agreement. If any law or judicial ruling renders any term or condition of this Agreement unenforceable, the remaining terms and conditions will remain in full force and effect.
Governing Law. Tulsa Federal Credit Union is located in Oklahoma and your Card is issued from Oklahoma irrespective of your residency or the jurisdictions in which you use the Card. This Agreement is entered into with you in the State of Oklahoma and shall be governed, construed, and enforced in all respects and all causes of action relating to terms or conditions of Card usage, or terms and conditions of this Agreement according to the laws of Oklahoma, without regard to its internal conflicts of law principles.

Travel Reloadable Prepaid Visa Card Agreement

This reloadable card Agreement (this “Agreement’) governs your and our rights and responsibilities concerning this reloadable card. Please read the Agreement carefully and keep a copy for your records. By accepting, activating or using your Card, you are agreeing to this Agreement.
Definitions: In this Agreement, the plural form of a word also means the singular, and vice versa. “You,” “your,” “yours” or “Cardholder” means the person who has received this Card. “Tulsa FCU,” “Credit Union”, “we,” “us,” “our,” or “ourselves” means Tulsa Federal Credit Union, the Credit Union providing this Agreement to you. “Account” or “Card” means the reloadable prepaid Card issued by us.
Card Qualifications: You must be a Credit Union member to purchase VISA Prepaid Cards from us. Cards may only be shipped within the United States. You must be a resident of the United States and at least 18 years or older to purchase the Card.
Before Using Your Card: Activate and register your Card by visiting www.tulsafederalcu.org. From our homepage, choose Prepaid Visa Debit Cards under Products, and then click on Register Card next to the image of your Card type. Read and accept the Disclosure and Terms and Conditions to be directed to our Prepaid Card site. Under Cardholder Login, select “Need a Login.” Fill in the information and select Continue. After the Card is registered, select the Activate Card option in the Account Activities dropdown menu. A Personal Identification Number (PIN) will be assigned once activation is confirmed. You may also call 855. 657.8588 to activate your Card and obtain your PIN. You will have the option to select your own PIN, via phone only, if you choose to do so.
Using Your Card: The Card will be personalized with the cardholder’s name when purchased through our website at www.tulsafederalcu.org or at any of our branches. The Personalized Card should be received in the mail within 7-10 business days from the date of completed purchase. The address listed for the primary cardholder must match the billing address for the debit/credit card used to purchase the Prepaid Card on the website. You may use any of our debit or credit cards to purchase or fund a Prepaid Card through the website.
Please sign the back before you use the Card. You are responsible for keeping the Card information, including the Card number, PIN, expiration date, and Card Verification Value code confidentially contained within your records. Do not write your PIN on or near the Card. Upon activation of the Card, for your protection and for regulatory purposes, we may request, and you agree to provide, some additional identification verification information such as your home phone number, date of birth, social security number, and zip code. We may use this data for a range of purposes, including but not limited to facilitating refunds if your Card is lost or stolen, enhancing usage at merchants that may require zip code authorization, and aiding in collection efforts in the event of a negative balance. If you do not wish to provide the data, or if the data you provide us cannot be appropriately verified, we reserve the right to refuse or cancel your Card, and reimburse you for the initial load amount. If the data you provide to us changes, such as an address change, we reserve the right to block access to your funds until your information has been updated. If you fail to update your information, we reserve the right to cancel your card. We may suspend your Card if we notice excessive use of your Card or other suspicious or potentially illegal activities. Access will be reinstated once we have notified you, clarified the situation, and rectified any problems.
Personalized Cards have an Allowance Feature that permits automatic funding to the Card from pre-existing stored payment methods on a weekly, bi-monthly, or monthly basis. To activate the Allowance Feature, you must register your Card at www.tulsafederalcu.org, and follow the instructions for activating the feature.
The Card is non-transferable. Please do not give your Card to anyone else to use. The primary cardholder, the Credit Union member, can order online up to 2 secondary Cards for family members or companions at the time of purchase. Secondary Cards are personalized with the cardholder’s name when ordered from our website or when purchased at a Credit Union branch. When purchasing a secondary Card for a young person with no card experience, be sure to go over all of the materials that accompany the Card. Secondary Cards will utilize the same prepaid balance as the primary Card for all transactions and fees. These Cards will have to be separately activated, and a separate PIN will need to be requested for each Card. Secondary Cards will function under the same terms and conditions as the primary Card. You are responsible for all authorized
transactions made with the secondary Card. Only the primary Cardholder, Credit Union member, can load funds to the account or access the Card information on our secured website. • IVR (Interactive Voice Response) Call Fee – a fee of $1.00 will be charged to the Card balance for each time a call to the IVR system is made. The first two calls per month are free.
• Customer Service (Live Agent) Fee – a fee of $3.00 will be charged to the Card balance for each call to a live customer service agent. The first call each month is free of charge.
• Inactivity Fee – An Inactivity Fee of $3.50 will be charged per month after 365 days of inactivity until the card balance reached zero.
• Express Delivery Fee – An Express Delivery Fee of $25.00 will apply to any Card that must have a rushed delivery when through the website or at a branch.

Card Limits. The daily spending limit for point-of-sale transactions at merchants is $5,000.00. You may not exceed more than 15 purchase transactions per day, up to the balance remaining on your Card.
• You are allowed up to 3 ATM withdraws per day, the total amount is not to exceed $1,530.00 at domestic ATMs or $1830.00 at international ATMs. The minimum ATM withdraw amount is $10.00.
• You are allowed up to 3 over the counter cash advances per day with the total amount not to exceed the balance on your card. The minimum over the counter cash advance is $25.00.
• Your Card is restricted to 4 value loads in a 7 day period. The total sum of value loads within that 7 day period is not to exceed $20,000.00.
• You may add up to 2 additional Cards for access to the same prepaid funds at the time of initial load.
• Personalized cards have an unlimited number of reloads with the exception of the 7-day restriction mentioned above.
Notice of Fees Charged by Others: If you use an ATM that we do not own, the owner of that ATM, or the network that processes your transaction, or both, may charge you a fee. Any such fee, which is generally called a “surcharge,” is not a fee that we charge you; however, we are authorized to pay any such surcharge from your account. Any such surcharge is in addition to any fees that we may charge you.
Non U.S. Transactions. If any Card transaction is made in a currency other than that loaded on the Card, the amount will be converted into the appropriate currency at an exchange rate on the day the transaction is processed. The exchange rate used to determine the transaction amount in US dollars is either a rate selected by VISA from the range of rates available in wholesale currency markets for the applicable central processing date, which may vary from the rate VISA itself receives, or the government mandated rate in effect for the applicable central processing date.
Liability for Unauthorized Use – Lost or Stolen Cards. If you believe the Card has been lost or stolen, or that someone has accessed or may access money from the Card without your permission, contact Cardholder Services immediately at 855.657.8588 or our Care Center at 800.256.5626 or 918.610.0200, or write to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK 74101, or visit one of our branches. You will not be liable for lost value on the Card if you satisfy the following conditions:
• You have immediately called us and reported the Card lost or stolen, which allows us to the status the Card.
• You have signed the Card on its reverse side in permanent ink.
• You inform us of the Card number and the approximate date of your last authorized use.
• You have not reported two or more incidents of unauthorized use to us in the preceding 12-month period.
• You report all facts of the loss or theft to us and you cooperate in our investigation.
If you permit someone else to use your Card, we will treat this as if you have authorized this person to use your Card and you will be responsible for any transactions initiated by said person.
Our Liability for Failure to Make Transactions. We may restrict access to the Card, temporarily or permanently, if we notice suspicious activity in connection with the Card, and we will notify you if we do so. We have no liability for restricting access to the Card because of suspected suspicious activity. Access will be reinstated if we determine that there has been no unauthorized use of the Card.
If we do not complete a transfer from the Card on time or in the correct amount according to our Agreement with you (other than for restrictions because of suspicious activity), we will be liable for the correct amount of the transfer. However, there are some exceptions. We will not be liable, for instance:
• If through no fault of ours, a merchant refuses to honor the Card;
• If through no fault of ours, you do not have enough money available on the Card to make a purchase;
• If the terminal or system was not working properly;
• If the Card is expired, damaged, or revoked;
• If the transaction information supplied to us by you or a third party is incorrect or untimely;
• If the Card has been reported lost or stolen;
• If we have reason to believe there is something wrong, for example, that the Card has been stolen;
• If circumstances beyond our control (such as flood or fire) prevent a transaction, despite reasonable precautions that we have taken;
• The merchant authorizes an amount greater than the purchase amount.
There may be other applicable exceptions as otherwise provided by state or federal laws.
PIN Protection. You agree to take all necessary steps to protect your PIN and never disclose your PIN to anyone. For security purposes, never write your PIN on the Card and never carry a record of your PIN on your person. If you permit someone else to use your Card and you give that person your PIN, we will treat this as if you have authorized this person to use your Card at their discretion for that transaction and any future transactions. You will be responsible for any transactions initiated by such person.
Your VISA Reloadable Prepaid Card will only allow 3 incorrect PIN tries in one day before the security system will lock you out, preventing access to any funds. The Card will remain locked for a 24 hour period. You may change your PIN after activation by calling Cardholder Services at 855.657.8588, or by visiting our website at www.tulsafederalcu.org.

Transaction Activity. A transaction activity report, which includes your balance and details the use of your Card, is available by calling Cardholder Services at 855.657.8588, writing to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101, visiting our website at www.tulsafederalcu.org, or visiting one of our branches. The transaction activity report must be requested by the Cardholder. The information is available to you 24 hours a day, 7 days a week. Your Card account number and original load value is required for authentication.
Error Resolution Procedures. In case of errors or questions about transactions arising from the use of your Card, or if any records you review show transactions that you did not make, call Cardholder Services at 855.657.8588, write to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101, visit our website at www.tulsafederalcu.org, or visit one of our branches immediately. We must hear from you no later than 60 days after the transaction appeared on the transaction activity report. We may, at our discretion, extend the period for a reasonable time. In order to help you with your questions, we will need the following information:
• Your name, address, phone number, and Card number.
• A description of the error or transaction you are unsure about and why you believe there is an error or why you need more information.
• The dollar amount and date of the suspected error.
Once the type of error is identified, we will mail you the appropriate paperwork to complete. No other action will be taken until the completed paperwork is received at the address specified in the documentation. Upon receipt of the completed paperwork, we will send you a confirmation letter and begin the investigation. We will determine whether an error occurred within 60 days. Our business days are Monday through Friday, and exclude holidays. If we need more time, however, we may take up to 45 additional days to investigate your complaint or question. If we decide to do this, we will send you a letter. Credit will be given only after it has been determined that it is warranted.
We will tell you the results after completing our investigation. If we decide that there was no error, we will send you a written explanation. You may ask for copies of the documents we used in our investigation by calling Cardholder Services at 855.657.8588.
Unauthorized Use of Your Card. You will notify us promptly within a reasonable time if the Card or PIN is lost or stolen. A replacement Card will be issued for the balance remaining on the Card at the time it is reported lost or stolen. To report a Card, PIN, or both lost or stolen, call us at 855.657.8588 or 918.610.0200 during business hours, or write to Tulsa Federal Credit Union Card Services, P.O. Box 267, Tulsa, OK, 74101-9939, or visit one of our branches.
A transaction is unauthorized if it is not initiated by you, you did not give permission to make the transaction, and you do not benefit from the transaction in any way. Any unauthorized transactions will be re-credited to your Card in 5 business days. If you do not tell us within 60 days after the transaction was posted to your account, you may not get back any money you lost if we can prove that we could have stopped someone from fraudulently using the Card if you had told us in time.
Special Notice to Visa Cardholders. If there is an unauthorized use of your Visa card that takes place on the Visa network, your liability will be zero ($0.00). This provision limiting your liability does not apply to non-U.S. ATM transactions or transactions using your PIN that are not processed by Visa. Additionally, your liability with respect to unauthorized transactions may be greater than this zero ($0.00) liability limit, to the extent allowed under Visa rules or applicable law. The Visa zero ($0.00) liability limit is subject to certain conditions. If we reasonably determine, based on substantial evidence, that you were grossly negligent or fraudulent in the handling of your card this zero ($0.00) liability limit will not apply. In any case, to minimize your potential liability you should notify us immediately of any unauthorized use but no later than 60 days after the transaction appeared on the transaction activity report. Disclosure of Information to Third Parties. We may disclose information to third parties about your Card or the transactions that you make:
• where it is necessary for completing the transaction;
• to verify the existence and condition of your Card for a third party;
• to comply with government agency or court orders;
• if you give us your consent; and
• as otherwise required by law.

Closure, Expiration, or Revocation of Card. Your Card may be unloaded at an ATM using a PIN, and may be closed by contacting Cardholder Services at 855.657.8588, visiting one of our branches, or by mailing a request in writing to Tulsa Federal Credit Union Card Services,
P.O. Box 267, Tulsa, OK 74101-9939.
Your Card is valid through the last day of the month of the expiration date displayed on your Card. The funds are available for a minimum of 5 years from the load/purchase date or until the entire prepaid value associated with the Card has been depleted, whichever comes first. If you do not deplete the balance by the expiration date of the Card and it has not been 5 years from the load/purchase date, a replacement card may be issued. (See Liability For Unauthorized Use – Lost or Stolen Cards section for contact information to request a replacement card). A renewal Card will automatically be mailed to the last known address as reflected on the cardholder record in the Card Processing System 30 days prior to the expiration date printed on the front of your Card. You must call Cardholder Services at the number on the back of the Card and activate your renewal Card when it is received. We encourage you to use the entire amount before the funds expire. The funds will expire in accordance with the statutory period under the unclaimed property laws in your state of residence if your address is known to us. When your state of residence is Oklahoma or cannot be identified, funds will expire in accordance with Oklahoma’s Uniform Unclaimed Property Act which has a statutory period of five years. In the event that you terminate your membership with the credit union, your card will be closed and any remaining balance will be refunded to you.
The Card is the property of us and we may revoke the Card at any time without cause or notice. You agree to surrender a revoked Card promptly upon demand. Upon revocation, any stored value remaining on the Card will be refunded to you less any applicable fees.
Amendments: We may change or add new terms to this Agreement at any time, including without limitation, new fees, fee increases or enforcement of rights and obligations under this Agreement. These changes will be posted on our website at www.tulsafederalcu.org, or as required by applicable law.
Assignment: We may at any time and without notice to you assign the Card, this Agreement or any of our rights or obligations under the Card or this Agreement to any person or entity. The person or entity to which we make any such assignment shall be entitled to all our rights and/or obligations under this Agreement, to the extent assigned.
Enforcement: This Agreement or any claim or dispute arising under this Agreement will be construed in accordance with and governed by the laws of the State of Oklahoma, together with specifically applicable federal laws governing this Agreement. If any law or judicial ruling renders any term or condition of this Agreement unenforceable, the remaining terms and conditions will remain in full force and effect.
Governing Law. Tulsa Federal Credit Union is located in Oklahoma and your Card is issued from Oklahoma irrespective of your residency or the jurisdictions in which you use the Card. This Agreement is entered into with you in the State of Oklahoma and shall be governed, construed, and enforced in all respects and all causes of action relating to terms or conditions of Card usage, or terms and conditions of this Agreement according to the laws of Oklahoma, without regard to its internal conflicts of law principles.

Gift Card Agreement

This prepaid non-reloadable gift card Agreement (this “Agreement”) governs your and our rights and responsibilities concerning this prepaid non-reloadable gift card. Please read the Agreement carefully and keep a copy for your records. By accepting, activating, or using your Card, you are agreeing to this Agreement.
Definitions: In this Agreement, the plural form of a word also means the singular, and vice versa. “You,” “your,” “yours” or “Cardholder” means the person who has received this Card. “Tulsa FCU,” “Credit Union”, “we,” “us,” “our,” or “ourselves” means Tulsa Federal Credit Union, the Credit Union providing this Agreement to you. “Account” or “Card” means the prepaid non-reloadable VISA Gift Card issued by us.
Card Qualifications: You must be a Credit Union member to purchase VISA Gift Cards from us. Cards may only be shipped within the United States. You must be a resident of the United States to purchase the Card.
Before Using Your Card: Activate and register your Card by visiting www.tulsafederalcu.org. From our homepage, choose Gift Cards under Products, and then click on Register Card. Under Log In, type in your Card Number and 3-Digit Code and click Log In. Go to Account Activities and select Register Card from the dropdown menu. After the Card is registered, select the Activate Card option. A Personal Identification Number (PIN) will be assigned once activation is confirmed. You may also call 866.902.6082 to activate your Card and obtain your PIN. You will have the option to select your own PIN, via phone only, if you choose to do so.
Using Your Card: Please sign the back before you use the Card. The Card is a non-personalized card. You are responsible for keeping the Card information, including the Card number, PIN, expiration date, and Card Verification Value, confidentially contained within your records. Upon activation of the Card, for your protection and for regulatory purposes, we may request, and you agree to provide, some additional identification verification information such as your home phone number, date of birth, social security number, and zip code. We may use this data for a range of purposes, including but not limited to facilitating refunds if your Card is lost or stolen. If you do not wish to provide the data, or if the data you provide us cannot be appropriately verified, we reserve the right to refuse or cancel your Card, and reimburse you for the initial load amount.
Your Card is issued by the Credit Union, pursuant to a license from VISA U.S.A., Inc., and may be used for purchases at most merchants where VISA Debit is accepted. Your Card allows you to access your prepaid funds at merchants who accept VISA cards worldwide. The Card can be processed as a Credit or Debit transaction. When paying as a Credit transaction, you may sign a sales receipt. When paying as a Debit transaction, you will need to enter your PIN. Some merchants may allow you to use your Card for cash-back on purchases, subject to the amount available on the Card. Your Card is neither a credit card, nor a deposit account insured by the NCUA. The Card may not be used for recurring payments, non-financial money exchange establishments, illegal transactions, and gambling establishments.
Merchants such as restaurants, hotels, mail order companies, and automobile rental businesses may attempt to submit authorizations against the Card for an amount greater than the actual purchase amount even though you will only be debited for the actual amount spent. If a merchant attempts an authorization greater than the balance remaining on your Gift Card, it may be declined. Automated fuel dispensers will automatically preauthorize a minimum of $99 (added on top of any merchant’s set preauthorization, which is usually $1). If you do not have the funds available on your Card, the transaction may be declined at the pump. It may take up to 24 hours before any unused amount is available to spend due to the way the transaction is processed with the preauthorization. Inside the gas station, however, the preauthorization is normally requested at the actual transaction amount.
This Card is not a credit card and may not be reloaded. The minimum load value is $25.00 and the maximum load value is $500.00. You may use it as often as you like while there is a balance remaining on the Card, and only up to the amount left on the Card. There is no minimum purchase amount. Purchases and transactions (including any transaction fees) will be deducted from your Card balance until the value reaches zero. If a transaction initiated by you exceeds the remaining balance on the Card, you must tell the merchant before completing the transaction. Any transaction attempted for more than the amount available on the Card may be declined. Therefore, you must know the amount available on your Card, and inform the merchant to process the transaction in that amount. You must pay the difference with another form of payment. If you are given value through the use of the Card greater than the balance remaining, you will pay us on demand the amount by which your transactions exceeded the balance remaining on the Card. You can check the balance or transaction history by calling 866.902.6082, by registering the Card at www.tulsafederalcu.org, by writing to Tulsa Federal Credit Union, P.O. Box 267, Tulsa, OK 74101 or by visiting one of our branches.
In the event that you need to return an item you purchased with your Card, the merchant will handle the item in accordance with VISA guidelines. The merchant may credit your Card (put the funds back on the Card) thereby increasing your available funds. The funds may take up to 7 business days to be credited to your Card.
Fees and Charges: The following account fees will apply as defined below:
• Purchase/Enrollment Fee – There will be a fee of $3.95 added to the load value on the Card so that the balance upon receipt of the card will not include that fee (for example, if a $100.00 Card is purchased, the amount charged to the purchaser’s account is $103.95 upon purchase and the Card balance is $100.00).
• Lost/Stolen/Replacement Fee – There will be a $5.00 fee to replace a lost/stolen/damaged card. Your Card must be registered through the website to qualify for a reissue.
• Inactivity Fee – An Inactivity Fee of $3.50 will be charged per month after 365 days of inactivity until the card balance reached zero.
